Are you currently guilded? If so, with whom? Hearth on Wipes
Why are you looking to leave? Lack of raiding caused by a lack of members.
What other guilds have you been in? Alliance Assassins, however Hearth on Wipes consisted mainly of the raiders of Hearth on Wipes. Alliance Assassins is more or less dead.
Why do you want to join Surreality? To see new raid content and have fun doing so.
What do you have to offer the guild? What are your expectations for us? A fun to be around player, who knows when to be serious when it's needed. I am very flexible, and I'm willing to change anything that is needed to benefit the raid.
Do you know any of our members? Who? I know Nathin (orc warrior, although, I'm not sure if he even plays anymore) and Elam. I also know a couple initiates. (Myshella and Clinttaurus)
What is your current talent distribution? How, or why, did you choose it? (Explain dubious talent choices. An acceptable answer to this question is not "because I was told to by a guild member in "x." Just like High School Math, show your work.)

I'm currently 0/13/58 Destruction Warlock.
I feel like I'm one of the original Destro warlocks. When I hit 80, affliction was the spec during naxx. Demonology seemed cool, but casting like a mage with fire spells was much more appealing to me. I say I'm one of the 'originals' because everyone flocked to destruction when T8 hit for ulduar, and it was definitely ahead of affliction at the time.

I have 2/2 in improved Healthstones because my guild is low on healers. We generally had less healers than we should have so I figured having an improved healthstone for everyone in the raid would be especially helpful for fights where the heals may come in slower for whatever reason. (mass aoe damage and healers are busy healing other raid members/tanks) or damage from say Decimate.

I've seen warlocks go 2/2 in Demonic Power (increases Imp cast speed) to have a DPS boost, and have 1/2 Destructive Reach (increased cast range, and reduced threat).
However, I believe this is wrong. I'm currently 1/2 Demonic Power and 2/2 Destructive Reach for very simple reasons. Increased range gives me the ability to hit the boss while being in very odd places and in a sense give myself more room to move. The threat reduction not only makes it easier on the start up for the tank(s), but also gives me a chance to just unload as many spells as possible before needing to drop threat, however by that time the tank has (hopefully) established enough threat to last them a good portion of the fight. In short, I can pewpew more, and won't have to worry about my threat as much. The ability to constantly cast with increased ranged, and reduced threat is a much better DPS increase than having my Imp shoot 0.25 seconds faster.

I'm spec'd into Improved Soul Leech (replenishment) because at the time during Ulduar, my guild was mainly a 10 man focused guild and had no replenishment sources. (as our Priest was disc due to lag issues) Our paladin was prot, and our hunter refused to go survival. We also didn't have any mages, nevermind frost mages.

What is your preferred raid role? If this role isn't available, are you willing to respec?

I prefer playing destruction, as it is definitely my most comfortable spec since I've been playing it for so long. My PvP spec was also destro, but since my arena partner and I haven't really been doing arenas, I recently dropped it.
I'm definitely willing to change specs, it may take a little time to learn the new spec but if the guild requires me to respec I will.

If you're applying as a DPS class, can you explain the basics of your rotation/priority system, and why you use it?

Before the encounter starts I lifetap to gain the spellpower buff from my glyph.
My rotation is more of a priority system. I keep the lifetap buff up, immolate on the target, and keep both conflagrate and chaos bolt on cooldown. If all is in check, I spam incinerate.
Immolate the target, Immolate is easily one of the best DPS spells a destro lock has, and gives a chance for my T10 to proc the damage buff. Curse of elements/agony/doom (In my guild, I'm the only source of the 13% magic damage debuff which is why I mentioned it)
I follow up immolate with a curse because due to lag/delay with the server, I am unable to conflagrate immediately. So i cover up that 'downtime' with a curse. If however, immolate is on the same target, there is no 'delay' for the game to realize immolate is on the target. So basically, if there's another warlock I can lifetap, curse, immolate and immolate immediately.
Conflagrate should always be on cooldown for the destruction spell haste buff. And chaos bolt is my hardest hitting spell which can't be resisted.
If the fight is expected to last several minutes, and there is a magic debuff, I will curse of doom. I will judge the rate at which the boss will die, and keep in mind if bloodlust has been used or not. If I think the boss will die in less than a minute, I will use switch to curse of agony.
If I have to switch targets, (eg. Toravon ice orbs) or have to move (eg. blood beasts on Saurfang) I will cast corruption on the boss so it can tick while I'm dealing with adds.

Surreality logs can be found at http://www.worldoflogs.com/guilds/4972/. You will be held to the standard of being able to put out numbers reasonably close to ours. Do you feel that this is something you're capable of doing? Do you have any applicable logs of your performance?

I might not be able to pull some of the numbers, but I'm definitely willing to try.
I am not afraid to push my limits when I need to, and I'm not prone to "tunnel vision" or "space/zone out" during extreme times of concentration. That only really happens if I get bored.
Sadly, I don't keep any logs of my DPS meters.
One of my mottos is to always do my best and exceed what is needed, mediocrity is not acceptable. This is not to say I can't have fun, this mostly applies during times when it really counts.

No one has their ideal gear, and getting that gear we want is part of the reason for raiding. Can you point out any problems you see in your own gearing at the moment, and explain your plans to address those issues?

Definitely trinkets.
Trinkets are extremely difficult for me to retrieve. There are many, many upgrades to the current trinkets I have. The problem is the raids which are required for those trinkets are hardly run, or just very unlucky RNG.
The Nevermelting Ice Crystal from heroic Pit of Saron I think sucks. The cooldown is not worth the trouble to add into fights, and the DPS increase would probably be extremely minor if I used it. No one wants to do specific dungeons so dungeon queues for it becomes impossible. (especially as a DPS)
it's hard to find a toc25 run that actually gets cleared before breaking up for whatever reason. Even if anub is downed, the trinket still has to drop, and I have to roll against all the DPS casters in the raid.

As for Mauradin's Spyglass, with the guild I'm currently with, I was one of the tanks. So my warlock has had minimal luck in actually getting into the gunship while the trinket actually dropped. again, a combination of bad RNG, while being on a different character.
All I can really do, is join raids and hope for the best. If it's a pug, get my lucky everything so I can roll high =]

Describe your previous raid experience. What is your favorite boss fight? Why?

Definitely the ulduar encounters. Namely, Hodir, Freya and Mimiron.
Each fight tends to have a lot going on, and so much to pay attention to, it's impossible to get bored. The chaos going on during hodir, with people yelling "frozen blows! frozen blows!" and looking around for haste lights and campfires, while needing to jump/move and dodge falling blocks makes the fight amazingly refreshing and unpredictable.
Freya is the same idea, switching targets and making elementals die at the same time, standing under mushrooms, dodging exploding mines, killing tons of adds leaves all the raid members with something to do and very little time to relax.
Mimiron is just pure hilarity and fun, with a lot of the same ideas as the previous bosses I explained earlier all wrapped into 4 phases.

What add-ons do you use? What add-on can't you live without? Why? Can you include a screenshot of your raiding ui?

My warlock uses:
Atlas loot, Bartender, button timers, button facade, Cartographer, Grid, minimap Button bag, omen 3, quartz, Deadly boss mods, recount, talented, tell me when and x-perl.

I cannot live without: bartender, button timers, button facade, omen 3, dbm and x-perl.
A lot of these are about making my UI pretty and smaller spell buttons, or just keeping track of buffs and debuffs crucial to maximizing DPS.

How many times a week are you willing to raid? When are you available?

I'm generally available Monday-Friday, as I work during the weekend.
I'm willing to raid a fairly decent amount, hopefully though I'd like to stay within 2-4 times a week.

How do you spend your time in-game outside of raid time?

Running circles in Dalaran, or ontop of the Org bank >_>
I do catch myself doing that when I'm trying to decide on what to do, however generally my time is spent doing a random dungeon on my different toons, profession dailies (eg. jewelcrafting on my deathknight) or just doing other raids I'm unable to do with the guild. (eg. VoA, weekly raid quests)

Tell us about a time you wiped your group or raid.

As a DPS, I generally can't think of something I did that solely caused the wipe of a group/raid.. One specific time comes to mind was during the early days of Icecrown, we hit the enrage timer on festergut, and I realized the number of times I didn't use my teleport circle to get back into position/cast corruption while moving. With that in mind I kind of realized if I had done those few things, the boss would be dead.
A time where I actually wiped a raid, was when I had my game sound off during Saphirrion in naxx. We were waiting on something, and I noticed someone's pet running toward the boss.. I soon realized it was my imp, but by that time he had already shot a fireball at Sapphirion, prematurely starting the fight and wiping the unprepared raid.

Best. AFK. Ever. My mic is rather quiet, so generally guildies have my volume turned up so they can hear me. I was a pug raid and told them in vent I had to go to the bathroom. I came back with about 20 jewelcrafting lenses focused and pointed at me.
Anything else?

Despite how serious I may come across to many, I'm actually a very fun individual. If I'm not doing anything and a guildie requires help that isn't too out of the way (eg. doing Zul'drak ring of blood) I'll fly over and help em out. I come with many quirks :P such as playfully yelling sparta!! when engaging as a tank, or giving funny nicknames to things. (abominations are boomers from Left 4 Dead, and the Ghasts that leap on people in Icecrown are noms that nom on people) I occasionally do impressions of bosses on vent, but depending on how well or poorly it's received, or asked, I will cease. =]
