Are you currently guilded? If so, with whom? Yes, SPACEBALLS THE GUILD. Casual/social guild.
Why are you looking to leave? I'm not really looking to leave as much as I'm looking for a guild that raids regularly on a schedule I can actually make.
What other guilds have you been in? On this server, Sadistically Insane (recently) and Syndicate of Chaos (beginning of Wrath), both of which were casual, 10 man guilds.
Why do you want to join Surreality?

Two main reasons. The first is that the people in Surreality are awesome, and the guild seems to have a good culture. The second reason is that I like (i.e., can make) the raiding schedule, and it's a relatively conservative schedule. The main thing that has kept me from raiding in Wrath is that I have a busy schedule, and I'm not OK with missing raids and impacting 24 other people by doing so.

Also, Surreality's recruitment post on the forums suggests that you are more interested in attitude and skill than purple pixels, so to speak, which is something I respect.

What do you have to offer the guild?

First I'll say what I do not offer, in the interest of modesty. :) First, I do not have a lot of raiding experience. I didn't play in Vanilla, didn't really raid much in TBC, and I haven't been playing for a large part of Wrath (Ulduar -> three weeks after 3.3). Although I am good with reading up on fights, situational awareness, and helping others understand mechanics when necessary, the most raiding I have done since I started playing in mid-TBC has been in the past two months (almost all PuGs).

I can offer a couple things to the guild. One is commitment — when I join a raiding guild, it takes a lot for me to leave. In fact, what has kept me from looking for a raiding guild before now is that my schedule has been fairly volatile with the semester starting, classes moving around, etc., but now that is not an issue.

I think the other thing I can bring that is useful to the raid is preparedness. I always have reagents, flasks, and food. I always read up on the fights. I am good with keeping up with my class and spec mechanics in the interest of maximizing DPS or healing capability (reading EJ, Shadowpriest.com, etc.).

What are your expectations for us? Maturity and organization. By "maturity" I mean handling guild issues (both pretty and ugly) in an adult manner. Keeping cool, basically. Well, for the most part. :)
Do you know any of our members? Who? Rosaly and I were in Syndicate of Chaos together for a brief period of time.
What is your current talent distribution? How, or why, did you choose it? 14/0/57. It's pretty much a cookie-cutter Shadow DPS build.
What is your preferred raid role? If this role isn't available, are you willing to respec?

I like both DPS and healing. I do not like healing in Wrath, though. The bursty nature of damage and healing is really not fun, in my opinion. That being said, I don't object to doing both, although I have much more experience (and gear) at this point for playing Shadow, at least in a raid environment.

Describe your previous raid experience. I already discussed this, but I'll add that my experience in ICC 10/25 is up to Saurfang. I always read up on fights before starting new content to a degree that I am comfortable I understand the mechanics involved, my responsibility, and where my obligation to do max DPS are subordinated by my obligation to help the raid kill the damn boss (killing Blood Beasts, for instance).
What is your favorite boss fight? Why? Too hard to choose. I'll just say VanCleef, since killing him on my first character was probably the most satisfying boss kill of all time. :)
What add-ons do you use? What add-on can't you live without? Why? I use a lot of addons, but I couldn't live without MSBT, PowerAuras, a good unit frame mod (using SUF currently), Grid, kgpanels, Quartz w/ MF-2, I think that's it. MSBT is just way too useful with its trigger system, PowerAuras is great for displaying various procs (or if Inner Fire fell off, oops), Grid is self-explanatory, Quartz is the only cast bar I'd ever use, and kgpanels keeps everything somewhat pretty. Of course, I also use DBM, Omen, and Recount.
How many times a week are you willing to raid? When are you available? 3 days a week, Monday - Thursday, evening/night. Friday - Sunday is really iffy for me, because I work in the evenings on those days and when I get home changes week-to-week.
How do you spend your time in-game outside of raid time?

It's going to be finishing Northrend quests (I leveled up healing instances), and doing dailies. I recently leveled Enchanting to 400, and I'm pretty poor now. :)

I PvP here and there, but I haven't really done so "seriously" since S2/S3.

Tell us about a time you wiped your group or raid. I used to play on a laptop, which pretty much limited me to 10 mans because it simply couldn't handle 25 mans. It had a tendency to lock up momentarily once in a while, where if I was running forward, I would keep doing so for five seconds or so until I regained control. I wiped a couple 25 man raids accidentally pulling a boss by running straight at him. XD Fortunately I now have a brand spankin' new desktop.
Best. AFK. Ever. I don't think I have any good AFK stories, sadly.
Anything else? This app is already too long, I think I'll leave it at that.
